Aging Japan Budget Map (prototype) · By Toshikazu Shiroiwa, Author & Advisor on Aging Japan
What Japan's national government spent on pensions, healthcare and long-term care in FY2025
That is about a day. One Japanese citizen added up Ministry of Health, Labour and Welfare programmes from the government's public review sheets. Japan's fiscal year runs from April to March.
These are the latest figures public as of November 2026. Actual spending includes supplementary budgets. Initial budgets and budget requests are plans, so they cannot be compared directly with actual spending. A budget request is what the ministry asked for, not an approved budget.

The money this map does not show
This map covers amounts recorded as national government programmes. Much of the premium income, local government spending and patients' out-of-pocket payments flow elsewhere.
Long-term care
This map (national programmes, FY2025 actual)
Total long-term care insurance cost (FY2026 budget basis)¥14.6 trillion
Healthcare
This map (national programmes, FY2025 actual)
National medical care expenditure (FY2023)¥48.1 trillion
Each pair of bars differs in year and scope, so dividing one by the other does not give a national share. They only show the difference in scale.
The Basic Pension includes tax funding, but this map shows it as paid from a special account. The special-account line does not mean premiums alone.

How this map was made
Data: 2026 review sheets of Japan's Administrative Project Review, published on 1 September 2026 and retrieved from the public API on 4 November 2026. The official 2026 CSV files were not yet available, so the figures still need checking against them. Spending up to FY2024 comes from the 2025 CSV files (as of 19 January 2026).
Fields: Programmes are sorted automatically by responsible bureau. Pensions: Pension Bureau. Healthcare: Health Insurance, Health Policy, Health Service and Pharmaceutical Safety bureaus. Long-term care: Health and Welfare Bureau for the Elderly. Some programmes therefore serve people of all ages, and about ¥0.24 trillion of long-term care contribution subsidies sits under healthcare.
Funding source: Based on the account where each programme is budgeted. Accounts could not be read for 34 programmes (about ¥0.2 trillion), which are counted under the general account.
Not included: Programmes of the welfare, disability and employment bureaus, and of other ministries.
Destinations: Only direct recipients of national funds are classified, by legal entity type and name, into five groups. Subcontracted and indirect payments are excluded. Direct recipients total 100.0% of actual spending. Classification is automatic, so individual errors may remain.
Translations: Programme names are the author's unofficial translations. Recipient names and programme descriptions appear only on the Japanese page, which names local governments and public bodies only. Private companies are not named.
No evaluation: This site does not judge any programme. Please check the original review sheets before drawing conclusions.
